Numerous studies, from as early as 2003 all the way to today, have shown that the bacterium Propionibacterium acnes (P.acnes for short), one of the main recognised causes of acne, pimples and breakouts in the skin, is becoming more and more drug-resistant. Oxygen-based products, such as benzoyl peroxide, on the other hand, work by creating reactive oxygen species (ROS) in the skin, which seems to get around the drug-resistance problem (because it’s not an antibiotic) but damages the skin in other ways. Noted as early as 1968 and reinforced again by studies in 2010 and today, ROS makes skin more comedogenic (likely to have pimples).

So, what gives?

A PROBLEM WAY TOO BIG TO IGNORE

Acne vulgaris affects around 85% of all people. And it’s not just teenagers. Many people continue to suffer from acne and pimples throughout various stages of their lives.

It’s caused by a few things: hyperkeratinization is a disorder in the cells that line hair follicles, and this is exacerbated by the skin over-producing normal oils known as sebum (hyperseborrhea) and the presence of the bacterium P.acnes. Acne is often the result of an infection, which is why pimples are red, sore and inflamed.

And, for many years, the go-to treatment was to blast the P.acnes with antibiotics. But that’s where a big part of the problem with treating acne today lies.


ANTIBIOTICS ARE CREATING DRUG-RESISTANT ACNE BACTERIA

The wide usage of oral and especially topical antibiotics to treat P.acnes has contributed to creating highly drug-resistant bacteria. In some cases, P.acnes’ drug resistance has been documented to be as high as 40%.

In a nutshell that means that P.acnes is so resistant to antibiotics today that treatments are failing. And the widespread use of antibiotics means that its drug resistance continues to grow.

So, naturally, the skincare industry has been looking at alternative ways to treat acne. And one of the most popular routes by far is to use oxygen-based ingredients, such as benzoyl peroxide, which isn’t an antibiotic but delivers a similar antimicrobial effect. The problem with oxygen is that it’s actually very damaging to your skin and tissue in other ways.


WHY OXYGEN-BASED PRODUCTS ARE MAKING THINGS WORSE

Apart from basically being free radicals (which cause pretty bad DNA damage and contribute to aging in the skin), the problem with the ROS that oxygen-based products employ to treat acne, is that it actually makes your skin more likely to form new pimples. Talk about your catch-22, right?

See, acne sufferers contain more specifically “charged” lipids in the outer layer of their skin, known as squalene. And, in this state, squalene is a very effective ROS scavenger (it neutralises oxygen radicals, almost like an antioxidant). But, if you apply an oxygen-based product to your skin, the ROS oxidises these lipids into squalene oxides. And it’s this squalene oxide that makes skin more comedogenic – likely to form new pimples, also called comedones.

In short, you need alternative treatments that are neither antibiotics nor produce oxidation in the skin.
